  1. Types of information collected by Canda Diacono and the purposes for which it is held
    I collect and process the following data:

    Personal Data
    This is data that identifies you or can be used to identify or contact you, such as your name, address, email address, phone number and payment details.  It also includes sensitive data such as your medical information.

    When contacting me, I store your full name, email, phone number and medical details for the sole purpose of communicating with you as it pertains to our patient / practitioner relationship, and in fulfilling my role as your chosen Medical Herbalist. 

    If you opt into it, you may also be sent my newsletter. You can opt out from this communication at any time.

    Non-Personal Data
    This type of data is anonymised and comprises information that cannot be used to identify or contact you.  It is gathered for the purposes of analysing aggregate data to better understand how the website is used and which pages are of most interest.

  2. Storage of information and security measures
    The personal data I collect can be both digital and paper.  All paperwork is kept in a locked, secure location.  All digital data is held on devices with appropriate security measures in place.  All email communications are conducted using a mail service with end-to-end encryption.  Appropriate security measures are in place to protect personal data from being accidentally lost, or used or accessed in an unauthorised way.

  3. Retention
    It is a legal requirement that I keep patient records for 8 years after cessation of treatment or death.  In the case of children and young people records are retained until their 25th birthday, or 26th if the young person was 17 at the conclusion of treatment, or 8 years after death.

  4. Your Rights
    You have the right:

  • to access your personal data; 

  • of erasure or ‘to be forgotten’;

  • to object to processing of your data; 

  • to rectification of personal data;

  • to be informed about the collection and use of your data 

  • to restrict processing of your data

  • of data portability;

  • rights relating to automated decision-making including profiling
