Patient Agreement

Consulting with a medical herbalist is a mutual commitment and I find it useful to outline our respective commitments, as this helps us both proceed with clarity.

Please read the information below carefully; it covers the patient registration, appointments, treatment, payment, and confidentiality.

Registration

  • The Patient Registration Form is a key part of engaging a medical herbalist and should be returned to me as soon as possible, and no later than 7 days before in the initial consultation.

Appointments

  • Return of the Patient Registration form secures the agreed appointment date and time.  

  • If you need to move or cancel your appointment, please give a minimum of 48 hours notice.  If less than 48 hours notice is given, full payment of the consultation may be charged at the herbalists discretion.

Treatment

  • You must provide complete information about your health at - or prior to - your appointment. This must include diagnoses made by other health professionals, and list any medications you are currently taking.  Failure to provide this information can result in incorrect diagnosis and/or inappropriate or ineffective herbal treatment.

  • It is important to contact your doctor and herbal practitioner if there is any major change in your health.  Should you have an adverse reaction to your herbal medicine, stop taking them and contact your herbal practitioner.

  • The treatment I provide is likely to involve dietary and lifestyle advice as well as the prescription of medicinal herbs.  

  • Herbal remedies may take the form of capsules, tinctures, teas or topical applications. 

Payment

  • Payment for herbal medicines will be requested once I have decided on your prescription/s; this is likely to be either at the end of your consultation or soon after, if further research is required.

  • Payment can be made by credit card, BACS or cash.

  • Herbal medicines will be dispensed and posted once full payment is received.

Confidentiality

  • All information shared by you is confidential, and your patient notes are anonymised and stored securely.

  • I may discuss your case anonymously with other health professionals in order to better support you.
